Sean “Diddy” Combs was transported by ambulance from FCI Fort Dix in New Jersey on Saturday morning for medical treatment after suffering a head injury, according to a report.
The rapper is 56 and is serving a sentence of 50 months tied to a prostitution related conviction, as reported by The New York Post.
TMZ identified the federal lockup as FCI Fort Dix and reported that Combs left the prison at about 1 a.m. He returned before 7 a.m., meaning he was outside the facility for only a few hours.

The circumstances surrounding the injury were not immediately clear. The report did not explain how Combs hurt his head, and it did not identify the medical facility where he received treatment.
The brief ambulance trip came after another difficult development for the Bad Boys Records founder. On Friday, a report said Combs had received a harsh penalty at the low-security prison following allegations involving favorable treatment and a contraband cell phone.
Prison officials sanctioned Combs for “disruptive conduct,” according to the report. As part of that punishment, he lost 41 days of good conduct time that he had accumulated while serving his sentence.
Good conduct time can be earned through good behavior and can lead to an earlier release. Losing those 41 days had a direct effect on the date Combs is expected to leave federal custody.

His original release date had been set for January 2028. Following the sanction, that date was postponed until March 2028, extending his expected stay at the New Jersey facility.
Combs allegedly possessed a contraband phone while behind bars. The report said he used the device to view sexually explicit materials that had been sent to him by an ex girlfriend.
The phone allegation was accompanied by claims that Combs had paid substantial amounts of money for access to special treatment. NBC reported that the alleged benefits included massages, alcohol, and special meals.
The report also claimed that Combs paid someone to make sure his toilet and shower were always clean before he used them. Those allegations helped place fresh scrutiny on his treatment inside the federal prison.
Combs is serving his sentence after being convicted in July 2025. The verdict followed a trial lasting two months that included graphic and sensational testimony about his conduct over a period of years.
That testimony described “freak off” sex sessions involving Combs, his then-girlfriend Cassie Ventura, and other women. According to the source report, some of those sessions continued for days and involved large quantities of lubricant.
Combs was also accused of hiring male prostitutes to have sex with the women during those gatherings. The report said the women were forced to take drugs while participating in the sessions.
The jury acquitted Combs of the more serious sex trafficking and racketeering charges. However, he was convicted on two counts of transportation to engage in prostitution, leading to the sentence of 50 months that he is now serving.
The reported medical trip adds another development to his incarceration, but key details remain unknown. The report did not reveal the cause or severity of his head injury, identifying only the short window during which he left and returned to FCI Fort Dix.
